In Memory of Gift
Use the form above to honour the memory of someone special who has died with an In Memoriam Donation to Hospice Care Ottawa. You decide whether to inform their loved one of your gift by email or mail if you provide that information (the donation amount will not be disclosed to the family). 
Select which location they were served at or pick Hospice Care Ottawa if unsure. If we served this individual, we always allocate to the location we served them at.

In Honour of Gift
Use the form above to mark a special occasion. Make an "in honour of" gift to celebrate a holiday, birthday, wedding, or other event. ​Your gift will commemorate a joyous moment, while supporting palliative and end-of-life care care. By making a donation, the special person you celebrate will receive an email from you, or card from Hospice Care Ottawa, acknowledging the kind gift you've made in their honour (your donation amount will not be disclosed to the recipient). ​

Matching Gift Programs
Many organizations have a program where they match any charitable contributions made by their employees. If your company has a policy, request a matching gift form from your employer and send it to us completed, signed and referencing your gift.  The impact of your gift can be doubled.  Some organizations also match donations made by retirees and/or spouses.  Check with your Human Resources Department.
