Hospice programs include day hospice, home support, family support, and residential hospice services. Hospice Care Ottawa delivers these services at three sites: May Court Hospice, La Maison de l’Est, and Ruddy-Shenkman Hospice. Position Overview:
As a member of an interdisciplinary team the Community Hospice Care Coordinator – In-Home Support coordinates and oversees the day-to-day functioning of community hospice care services and programs provided by Hospice Care Ottawa. Working collaboratively with other Care Coordinators, this position will manage a case-load of clients/caregivers and oversee a team of volunteers, to coordinate the administration of programs such as In-Home Support (Volunteer visits), Day Hospice and Caregiver Support.

Working collaboratively with other Care Coordinators, this position will manage a case-load of clients/caregivers and oversee a team of volunteers, to coordinate the administration of programs such as In-Home Support (Volunteer visits), Day Hospice and Caregiver Support.
Key Responsibilities of the In-Home Support Program Care Coordinator:
- Manages a team of visiting volunteers & a case load of clients/caregivers
- Provides telephone intake and referral to clients and caregivers calling from the community
- Makes home visits to assess appropriate clients and caregivers and refer to the Hospice Programs
- Determines best suitability and assigns volunteers to clients/families for In-Home Support visits
- Acts as patient advocate where needed and desired by the patient and family
- Participates in patient conferences with other agencies and team members
- Meets with clients and caregivers on a regular basis, ensuring that their needs are being sufficiently met and, in a way, which is acceptable to them
- Liaises with volunteers regularly to support their work with clients and caregivers
- Organizes quarterly program support meetings for the Volunteer team
Required Education and Experience:
- Registration as an RN with the College of Nurses Ontario
- Certification in Hospice Palliative Care Nursing (C)anada (CHPCN(C)) preferred
- LEAP Certificate (Learning Essential Approaches to Palliative Care), or willingness to complete within 1 year of hire
- At least 3-years’ experience in hospice palliative care/and or background in community palliative care nursing
- Experience volunteer coordination and management is an asset
Required Skills:
- Understanding and demonstration of Ethics as they relate to Palliative Care
- Demonstrated experience in client assessment and patient advocacy
- Superior communication skills - both oral and written
- Strong computer skills in Microsoft and experience in InfoAnywhere or similar EMR
- Strong knowledge of community resources
- Demonstrate a commitment to on-going education and professional growth to maintain current knowledge in the changing face of both hospice palliative care and bereavement
- Bilingual in both official languages preferred
This position will also:
- Require travel within the Ottawa-area; access to reliable transportation needed
- Share On-Call Duty rotation that includes evenings, weekends and holidays
- Serve as back-up support to the Care Coordinator – Day Hospice Program when needed
