Admissions Coordinator
Full-time, 37.5 hours/week
Bilingual Preferred

Position Overview:

The Admissions Coordinator is the first point of contact for information regarding hospice programs and services and follows through initiating the admission process for Hospice Care Ottawa. The Admissions Coordinator is responsible to process referrals from the time of referral into appropriate Hospice programs including residence admissions and community services. The Admissions Coordinator ensures that each client is properly onboarded and that patient flow and service levels are maintained. The Admissions Coordinator is also the main support to the Scheduling Office and will be included in a regular On-Call rotation duty. Duties related to patient and family support with admissions are also performed and is the backup for the Scheduling Coordinator.

Required Education and Experience:
  1. Registration with the College of Nurses of Ontario; RN Registration is preferred, however applicants who do not possess an RN designation but possess the experience and required skills required may be considered.
  2. At least 3-years clinical experience in hospice palliative care and experience working with clients in the community with complex chronic illness
  3. Superior knowledge on the progression of chronic and palliative /end-of-life diseases as it would relate to a patient’s eligibility for admission to Hospice
  4. Demonstrated ability to perform advanced clinical assessments using evidence-based knowledge
  5. Experience communicating with clients and families during crisis
​Required Skills:
  1. Bilingual in both official languages preferred; Candidates who do not meet this language requirement may still apply and would be considered should there be no qualified applicants who meet this requirement
  2. Strong decision-making ability
  3. Advanced medical terminology skills
  4. Ability to work under pressure
  5. Ability to problem solve and adjust to rapidly changing priorities in a deadline driven environment
  6. Exceptional planning, organization, task prioritization and coordination
  7. Ability to work collaboratively with health professionals inside and outside of the Hospice team
  8. Possess and demonstrate strong customer service skills
  9. Excellent communication skills
  10. Strong computer skills in Microsoft and experience in InfoAnywhere or similar medical software
  11. Quick and accurate data entry skills

This position will also:
  • Serves as backup to the Scheduling Coordinator and serves as their replacement in their absence
  • Works collaboratively with the Scheduling Office to forecast staffing needs
  • Collaborates with the Scheduling Coordinator to ensure staffing levels can support patient needs
  • Included in On-Call Duty rotation that includes evenings, weekends and holidays

  • Ability to make timely clinical judgement using established criteria as a guide as it relates to determination of eligibility of patients and admission planning process

Communication and Relationship Management:
  • Demonstrates strong communication skills and ability to interact with multiple internal and external partners and individuals in a tactful and professional manner
  • Collaborates with the Scheduling Coordinator, On-call nurses, Team Leads and residence management to maximize the care able to be provided in residence

Leadership:
  • Exhibits strong interpersonal and leadership abilities as evidenced through the ability to professionally work through situations of conflicts
  • Demonstrates personal maturity as evidenced by the ability to work in a self-directed manner; seeks appropriate clarification on issues that fall outside one's professional scope of practice

Business Skills:
  • Demonstrates an openness to embrace new technologies to improve day-to-day efficiencies

Reporting Relationship:
Reports to the Director of Residence Care

Position Status: Full-time, Permanent, 37.5 hours per week

Position Schedule: Monday-Friday days, rotational On-Call duties that include evenings, weekends and holidays

Compensation: Salary; Eligible for Group Benefits (Health/Dental, Life Insurance & Disability) after 90 days.  Eligible for paid personal/sick leave

Location of Work: Ottawa or Kanata.  Will require flexibility for remote work if required

Travel: Must be able to travel in Ottawa between Hospice Care Ottawa's sites when required

Language of Work: Bilingual in both official languages preferred; written correspondence will be in English
